Scaffolding RAMS are reviewed against a well-established industry standard, which means gaps are obvious. The most common rejection is a document that does not make clear whether the scaffold is TG20 compliant or requires a bespoke design — and if bespoke, who designed it and to what loading class.

The lifting element people miss
Where scaffold materials are lifted by crane or telehandler — loading bays, temporary roofs, encapsulation panels — the lifting element falls under LOLER 1998 and needs planning by a competent person alongside the scaffold RAMS.
